Praziquantel

Zambia Specific Information

  • Available formulation in Zambia: Praziquantel Tablet: 150 mg; 600 mg.
  • Second line treatment of tapeworm (T. saginata and T. solium): albendazole preferred, but if pregnant, use praziquantel 10-20mg/kg x 1.
  • Bilharzia (S. haematobium and S. masoni): praziquantel 40mg/kg x 1.

USUAL ADULT DOSING

  • Schistosomiasis: 60 milligrams/kilogram/day PO in 3 divided doses as a 1-day treatment.
  • Clonorchiasis and opisthorchiasis: 75 mg/kg in 3 divided doses (must be administer at 5 hour intervals).
  • Cestodiasis (Tapeworm): 10mg/kg PO x 1 dose (usual adult dose 600 mg x1).

ADVERSE DRUG REACTIONS

DRUG INTERACTIONS

  • Macrolides: may increase praziquantel serum concentrations.
  • Phenytoin: praziquantel AUC was reduced by 70%. Use with caution and monitor for clinical failure. Increase dose based on clinical response.
  • Rifamycin: may significantly decrease praziquantel serum concentration. Use with caution and monitor for clinical failure. Increase dose based on clinical response.
  • Azole antifungals: may increase praziquantel serum concentrations.
  • HIV protease inhibitors: may increase praziquantel serum concentrations.
  • Nevirapine: may significantly decrease praziquantel serum concentration. Use with caution and monitor for clinical failure. Increase dose based on clinical response.
  • Efavirenz: may significantly decrease praziquantel serum concentration. Use with caution and monitor for clinical failure. Increase dose based on clinical response.
  • Corticosteroids: may significantly decrease praziquantel serum concentration. Use with caution and monitor for clinical failure. Increase dose based on clinical response.
  • Phenobarbital: may significantly decrease praziquantel serum concentration. Use with caution and monitor for clinical failure. Increase dose based on clinical response.
  • Carbamazepine: may significantly decrease praziquantel serum concentration. Use with caution and monitor for clinical failure. Increase dose based on clinical response.

SPECTRUM

Very broad activity that includes S. mansoni, S. haematobium, S. japonicum, intestinal cestodes (e.g., Diphyllobothrium latum, Taenia spp., Hymenolepis, and Dipylidium caninum)

PHARMACOLOGY

Pharmacology

COMMENTS

Well-tolerated agent, active against many trematodes and cestodes. Albendazole more effective for neurocystcersosis. Potential for many drug-drug interactions.
